153 Rockridge Drive, Unit 103, in Blumenort, MB.​
Parking is available in front of the building.
Arrival
Make yourself comfortable in our waiting room; your therapist will meet you for your appointment. Please wear comfortable clothing that allows you to move freely.
First Visit
Your first visit will primarily consist of reviewing your patient history. Please fill out the form prior to treatment to allow time for a full assessment. Fill out and Print the Patient History Form HERE. For more information about your First Visit click here.
Treatments
Follow-up visits will include a shorter evaluation and longer treatment.​Treatment sessions are 50-70 minutes in length. ​Please wear comfortable clothing.

Please have payment ready prior to arrival.
* We use Square for our card transactions - Debit Fee is an additional $2 and Credit Card Fee is $3.50.
Insurance
As a member of the Manitoba Association of Osteopathic Manual Therapists (MAOMT) and an affiliate member of the Ontario Association of Osteopathic Manual Practitioners (OAO), FMT inc. is covered by all major insurance companies such as Blue Cross, Canada Life, Great West Life, Standard Life, Sun Life, Manulife, Empire Life, Green Shield, Equitable Life, Chambers of Commerce and others. Please check with your provider. Please note we do not provide Direct Billing Services.
Cancellation
Please give a 24 hour cancellation notice by phone.
Missed, forgotten or disregarded appointments will be charged the session fee of $132.30 including GST. An alternative would be to send a friend or family member to attend your session. Please provide the name of the person attending by phone or email and if it is their first time they will need to fill out the Patient History Form prior to their arrival.
Medical
Please provide any medically relevant documents.
Minors
Did you know that Osteopathy Treatments can be beneficial for for all ages? Minors are to be accompanied by a parent or guardian.
